The Foundation for Staying Healthy
Stratum LIFE (Living Independently For the Elderly) was built on the belief that nothing provides the foundation for staying healthy like comfort, security, and peace of mind you can only enjoy in your home. Based in south Bradenton, Stratum LIFE will serve fragile Manatee and Sarasota County seniors in need of health services, adult daycare, and daily supportive care. The Center will serve aging seniors in need of primary and specialty medical services, nutrition, therapies, medication monitoring, and personal and supportive care delivered through an activity-rich, adult day care environment.
The primary benefit of the Stratum LIFE program is that it supports aging seniors’ desire to remain in their own homes, instead of moving into a nursing home environment. Patients receive a home safety review and then the Center provides amenities including emergency call systems, safety bars, ramps, etc. to ensure seniors can remain safely in their homes.
Stratum LIFE embraces a model called PACE (Program for the All-Inclusive Care of the Elderly). PACE programs are funded by the Centers for Medicare and Medicaid Services and paid by the state through Medicaid. The program pays on a per-person basis and offers “a comprehensive range of care and services to enable elderly individuals to continue living safely within their communities for as long as possible,” according to an analysis published by the National Institutes of Health. Services are delivered at the Stratum LIFE Center, as well as through home deliveries to patients to support nutrition, medication, and equipment needs.
PACE operates under the philosophy that seniors with chronic care illnesses are better served within their community whenever possible. Participants must enroll and the requirements are very simple: Be age 55 or older, live in the service area of Manatee or Sarasota County, meet the requirements of nursing home level of care, and with the Stratum LIFE Center interventions, the patient can remain safely living in their own home and community.
